The Foundation's Roadmaps to Health Community grants initiative was designed to identify, support, and disseminate models of cross-sector coalitions working to improve health by taking action to address the social and economic determinants of health, such as education, employment, income, community safety, and family and social support.This grant supports Community Catalyst to manage and lead the Roadmaps to Health Community Grants program and to deliver non-lobbying technical assistance on policy advocacy to the thirty Roadmaps to Health Community grantees. In this third year of the program, Community Catalyst will focus on synthesizing lessons learned from the community grantees and translate those lessons into story ideas, web content and other mechanisms for sharing like conference sessions and issue briefs. Community Catalyst will work collaboratively with the University of Wisconsin Population Health Institute and other County Health Rankings & Roadmaps partner organizations on strategies to support communities working to improve health and by collecting and sharing stories about community's taking action. No RWJF funds will be used for lobbying. Community Catalyst has retained counsel to assist them in complying with this restriction and assisting Roadmap grantees with legal compliance questions.
